Responsibilities- Provide comprehensive primary care services in an outpatient clinic setting to adult and senior patients, focusing on those with multiple chronic and complex diseases such as heart disease, diabetes, and hypertension.
- Utilize a value-based care model to enhance patient satisfaction, improve health outcomes, and reduce unnecessary hospitalizations.
- Manage a patient panel with an average of 10-12 patients per day and a maximum panel size of 650 patients, ensuring each patient receives personalized and attentive care.
- Lead and collaborate within a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership, to coordinate and optimize patient care.
- Participate in activities promoting holistic health for patients, including coordination of wellness activities and community engagement initiatives.
- Maintain accurate and timely documentation in compliance with organizational and regulatory standards.
- Participate in a call schedule, which may vary depending on the number of providers in the market (e.g., 1:32 weeks if 32 physicians).
- Demonstrate proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board certification or board eligibility in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is considered an asset but is not required.
- Active, unrestricted medical license to practice medicine in Alabama.
- Current and valid DEA registration.
- Proficient in performing essential office-based procedures as outlined in responsibilities.
