Primary Care Physician (MD/DO, FM/IM) - Summary- Full-time, outpatient, value-based care role caring for senior patients with complex health needs

- Provide comprehensive primary care to a panel of up to 650 senior patients
- Manage an average of 10-12 patients per day in an outpatient clinic setting
- Lead and coordinate care with a multidisciplinary team, including MA, scheduler, and referral specialist
- Participate in value-based care initiatives, focusing on patient satisfaction, HEDIS compliance, and reducing hospitalizations
- Perform office-based procedures: suturing, joint injections, skin biopsies, basic wound care
- Contribute to a supportive and inclusive clinic culture
- Participate in a call schedule based on market physician count (e.g., 1:32 weeks with 32 providers)
- Base salary: $260,000 with total earning potential over $312,000 including performance bonus
