Primary Care Physician (MD/DO, Family Medicine or Internal Medicine) - Summary- Full-time position with a leading value-based care delivery organization
- Focus on quality, not quantity, with manageable patient loads
- Collaborative, team-based care model with strong support infrastructure
- Located in the vibrant community of Lafayette, LA
- Provide comprehensive primary care services to adult patients as a Primary Care Physician (MD/DO, Family Medicine or Internal Medicine)
- Conduct thorough patient assessments, diagnose and manage acute and chronic medical conditions
- Develop and implement individualized care plans in coordination with interdisciplinary teams
- Engage in value-based care initiatives, focusing on quality improvement and patient satisfaction
- Participate in transitional care and outreach programs for high-risk patients
- Utilize EMR systems for accurate and timely documentation of patient care
- Consult and collaborate with APRNs, RNs, and a dedicated Quality Improvement team
- Receive and act on regular feedback regarding quality measures, diagnostic accuracy, and hospitalization rates

- M.D. or D.O. degree required
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted Louisiana medical license
- Valid DEA and CDS licenses (or ability to obtain)
- Preferred: 1+ years clinical experience in adult, family, or geriatric care
- Preferred: Basic Cardiac Life Support (BCLS) certification
