As a Primary Care Physician (MD/DO), you will work within a value-based care (VBC) model that prioritizes preventive health, chronic disease management, and continuity of care. Primary Care Physician (MD/DO) - Summary- Full-time outpatient primary care position serving adults age 65 and older
- Practice in a value-based care environment with a low daily patient census
- Opportunity to build strong, meaningful relationships with patients
- Work collaboratively with a multidisciplinary team including Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, and pharmacy and lab support staff
- Comprehensive onboarding and training in value-based care principles and EMR systems
- Provide comprehensive preventive and chronic disease primary care to a panel of adults aged 65 and above, with a focus on Medicare Advantage patients
- Manage a daily patient census of approximately 12–15 patients, allowing for 20–40 minute visits per patient
- Participate as an integral member of the integrated care team to coordinate patient care, including referrals, social work, and pharmacy needs
- Utilize electronic medical records (EMR) for documentation and care coordination
- Engage in ongoing professional development, including a 3-week comprehensive onboarding program with training in value-based care, documentation best practices, and integrated care workflows
- Monitor and improve patient health outcomes by adhering to evidence-based guidelines and quality metrics
- Actively contribute to a culture of collaboration, communication, and continuous improvement

- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Geriatric experience preferred, but not required
- Prior value-based care or HEDIS experience is a plus, but not mandatory; training will be provided
