As a Primary Care Physician (MD/DO), you will have the ability to form strong, long-term relationships with your patients, free from RVU constraints. Primary Care Physician (MD/DO) - Summary
- Full-time Primary Care Physician (MD/DO) position in a value-based organization serving adults 65+.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of adult patients, primarily age 65+.
- Conduct thorough patient assessments, chronic disease management, and preventive care visits.
- Manage a balanced daily patient census (approximately 12–15 patients per day).
- Spend 20–40 minutes per patient visit to ensure thorough and personalized care.
- Collaborate with an integrated care team including NP/PA, medical assistants, social workers, and pharmacy/lab support.
- Coordinate referrals and ensure continuity of care for complex cases.
- Participate in quality improvement initiatives and value-based care training programs.
- Maintain detailed and accurate clinical documentation in compliance with regulatory requirements.

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active and unrestricted medical license (MD or DO) in the state of practice.
- Experience with geriatric populations and Medicare Advantage is preferred.
- Familiarity with value-based care principles and HEDIS metrics is a plus, but not required—training available.
- Strong clinical, organizational, and communication skills.
