Primary Care Physician (MD/DO) – Summary
- Deliver value-based primary care to Medicare patient populations in a collaborative, patient-centered environment.
- Work with an innovative care team model that prioritizes quality over quantity of services.
- Access to on-site labs, support staff, and technology-enhanced medical practice tools.

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of adult and geriatric patients.
- Conduct regular and diagnostic visits, with appointment durations of 20 minutes for standard visits and 40 minutes for new patient or full diagnostic appointments.
- Collaborate with a multidisciplinary care team, including scribes, medical assistants, and social workers, to coordinate patient care and enhance clinical outcomes.
- Utilize advanced electronic medical record (EMR) systems, such as Canopy and Greenway, to document patient encounters and manage care.
- Participate in value-based care initiatives focused on preventive medicine, chronic disease management, and reducing unnecessary hospital admissions.
- Engage patients in health education, wellness activities, and community programs such as exercise, yoga, dance, and wellness events.
- Ensure compliance with all regulatory and ethical standards in the practice of medicine.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license (no probationary status).
- Fellowship training in Geriatrics is preferred but not required.
- Residency completed; residents are not eligible for this role.
