Our primary care centers operate Monday through Friday, offering manageable patient volumes to ensure quality interactions and comprehensive care. Providers see an average of 13-16 patients per day, with sufficient time allocated for both routine and diagnostic visits. The care model emphasizes preventive care, patient education, and community engagement, creating a positive impact on patient health and satisfaction.
Primary Care Physician (MD/DO) - Summary- Deliver value-based medical care to Medicare populations in a collaborative, patient-focused setting.
- Participate as a member of a large, multidisciplinary care team with access to scribes, medical assistants, and social workers.
- Utilize in-house labs and advanced EMR systems (Canopy and Greenway) to support patient care.
- Engage in community outreach and patient wellness activities to promote preventive health.
- Provide comprehensive primary care, including preventive, diagnostic, and therapeutic services to adult and senior populations.
- Conduct regular and full diagnostic visits, ensuring thorough evaluation and patient education.
- Manage a patient panel with an average daily volume of 13-16 patients to maintain high care standards.
- Collaborate with a care team (scribe, MA, social worker) to coordinate patient care and follow-up.
- Utilize EMR systems for accurate documentation and access to specialist consults via RubiconMD.
- Participate in quality improvement initiatives focused on reducing hospital admissions and improving outcomes.
- Support patient access through clinic-based transportation services and community room activities, such as exercise classes and wellness events.
- Engage in ongoing professional development and participate in team meetings.

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted medical license in the state of practice.
- Fellowship training in Geriatrics is preferred but not required.
- No residents; candidates must be fully licensed and able to practice independently.
