Primary Care Physician (MD/DO) - Summary
- Outpatient-based primary care role serving a senior (65+) population
- Full-time contract with a minimum three-month commitment

Duties & Responsibilities
- Provide comprehensive primary care services to patients aged 65 and older in an outpatient setting
- Conduct patient assessments, routine exams, chronic disease management, preventive care, and minor procedures
- Manage a daily patient census of 12-18 individuals, ensuring high-quality, patient-centered care
- Work collaboratively with a full clinical and administrative support staff
- Utilize a clinical care model focused on reducing emergency room visits and hospital admissions by emphasizing preventive health and proactive management of chronic conditions
- Document all patient interactions and follow best practices in clinical protocols
- Participate in care coordination and interdisciplinary team meetings as needed
- Maintain compliance with all relevant state and federal healthcare regulations

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Board Certified in Internal Medicine (ABIM) or Family Medicine (ABFM)
- Active Colorado medical license required
- Current DEA registration in Colorado, or willingness to transfer
- Basic Life Support (BLS) certification
- Experience with Medicare Advantage plans preferred
- Commitment to a minimum three-month contract
- Excellent interpersonal, organizational, and communication skills
