We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our growing healthcare team, focused on delivering high-quality, patient-centered care to seniors aged 65 and older in Baltimore, MD. As a Primary Care Physician, you will have the opportunity to practice at the top of your license within a value-based care model designed to maximize both patient outcomes and provider satisfaction. Our organization is committed to providing a concierge level of service to seniors who may not otherwise be able to access such comprehensive care.
In this full-time Primary Care Physician (MD/DO) role, you will benefit from a supportive care team and a state-of-the-art 10,000 square foot medical center equipped with labs, EKG, and echocardiogram services. The care model is designed to allow physicians to focus on quality over quantity, with low daily patient volumes averaging 10-13 patients per day and a maximum patient panel of 400. Charting is completed during office hours, enabling a strong work-life balance and ensuring providers have time for personal pursuits outside of clinical responsibilities.
Our team approach means each provider is supported by dedicated medical assistants, referral specialists, nurses, case managers, social workers, outreach specialists, and scheduling staff. The center is also enriched by programs that foster patient engagement, including cooking classes, chair yoga, exercise communities, and other social activities for seniors.
Patient access is further enhanced through transportation services coordinated by care team representatives, ensuring that all patients can attend appointments and specialist referrals with ease. As a Primary Care Physician (MD/DO), you will play a key role in shaping the patient experience and ensuring the highest satisfaction scores, often reflected in our consistently positive feedback.

Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive primary care to a diverse panel of senior patients
- Practice in an outpatient setting with a focus on value-based care
- Manage a patient panel of up to 400 patients, with an average of 10-13 patients seen per day
- Complete all necessary charting and documentation during office hours
- Collaborate closely with a dedicated care team, including medical assistants, nurses, case managers, and social workers
- Participate in community health initiatives and patient engagement activities
- Take part in a rotating call schedule (1:8 weeks), with an average of 3-5 calls per week
- Contribute to maintaining high patient satisfaction and quality care outcomes
- Participate in ongoing training and professional development
Salary & Benefits
- Competitive base compensation starting at $320,000+ annually, with additional incentives up to 28% of base pay
- Sign-on bonus
- Eligibility for partnership after three years (no buy-in required)

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine (IM) or Family Medicine (FM)
- Active and unrestricted medical license or eligibility for licensure
- Strong computer skills and comfort with electronic health records
- No current legal or professional disciplinary cases, impairment, or violations (including Medicare compliance)
- Open to hiring residents completing training in the near future
