Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive outpatient primary care services using a value-based care model
- Focus on senior patients with multiple chronic conditions
- Work as part of a multidisciplinary care team with strong support from medical assistants, schedulers, referral specialists, and leadership

Duties & Responsibilities
- Provide primary care services to a diverse patient population, with an emphasis on seniors managing 3-5 complex diseases such as heart disease, diabetes, and hypertension
- Engage in proactive, preventive care utilizing evidence-based guidelines and best practices
- Lead and collaborate with a dedicated care team to ensure comprehensive care coordination and high-quality outcomes
- Utilize advanced office procedures, including suturing, joint injections, skin biopsies, and basic wound care, as needed
- Maintain accurate and timely documentation in electronic medical records
- Participate in ongoing quality improvement initiatives and professional development opportunities
- Foster strong, long-term relationships with patients and their families, emphasizing respect, trust, and clear communication
- Contribute to a culture of wellness, inclusivity, and patient empowerment within the clinic and community

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics (FM/IM/Geriatrics)
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures: suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is advantageous but not required
