Our interdisciplinary approach is focused on value-based outcomes, working with a small patient panel to ensure personalized attention and continuity of care. Physicians benefit from comprehensive support teams, including medical assistants, schedulers, and referral specialists, to optimize the patient and provider experience. Administrative time is built into your schedule, and there are no nights or weekend shifts, supporting both patient access and provider well-being. The clinic provides in-house consulting specialists in cardiology and podiatry, as well as holistic health services such as acupuncture, tai chi, yoga, and nutritional counseling, all aimed at proactive senior care. Free door-to-door transportation is available for patients who need it, further reducing barriers to care.

Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care services to a panel of senior patients, managing 3-5 chronic conditions per patient
- Conduct outpatient visits (approximately 18 patients per day)
- Develop and implement individualized care plans focusing on prevention and chronic disease management
- Work closely with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Collaborate with in-house consulting specialists and holistic care providers
- Ensure high-quality patient experiences through coordinated, value-based care
- Participate in care team meetings and continuous improvement initiatives
- Utilize administrative time included in your schedule for documentation and care coordination
- Promote patient education and engagement in their health plan

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Residency trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatrics
- Passion for value-based, patient-centered care
