Primary Care Physician (MD/DO) - Summary
- Deliver preventive and primary care services to an adult population, primarily those 65+ on Medicare Advantage plans.
- Practice within a value-based care model that emphasizes patient outcomes and satisfaction.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of 12-15 patients per day, focusing on chronic disease management and preventive health.
- Conduct thorough patient assessments, develop personalized care plans, and perform follow-up visits to monitor and adjust treatment as needed.
- Engage in longer patient visits (20-40 minutes) to build strong, trust-based relationships and address complex health needs.
- Collaborate with nurse practitioners, physician assistants, medical assistants, social workers, pharmacy, and referral coordinators to deliver integrated care.
- Document accurately and efficiently in the electronic medical record (EMR), with training and support for system optimization.
- Participate in value-based care training, quality improvement initiatives, and ongoing professional development to enhance patient care delivery.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience in geriatrics or working with older adult populations is preferred.
- Value-based care or HEDIS experience is a plus, but training will be provided for those new to these models.
- Active and unrestricted medical license or eligibility to obtain one.
