Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide direct primary care to a patient panel of adults aged 65 and older, with a focus on Medicare Advantage populations.
- Spend 20-40 minutes per patient visit, allowing you to address complex needs and foster strong provider-patient relationships.
- Work alongside nurse practitioners/physician assistants, medical assistants, social workers, and in-house lab and pharmacy support.
- Participate in an integrated, team-based care model to coordinate referrals and ensure comprehensive care.
- Utilize electronic medical records (EMR) efficiently and adhere to documentation best practices.
- Engage in value-based care strategies, including preventive screenings, chronic disease follow-up, and population health initiatives.
- Attend and complete a 3-week onboarding program covering value-based care principles, EMR optimization, and workflow integration.
- Contribute to a culture of continuous improvement, quality, and patient satisfaction.

Qualifications & Requirements
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience in geriatric medicine is preferred but not required.
- Familiarity with HEDIS measures considered an asset.

- Strong communication and teamwork skills essential.
