Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to a predominantly senior patient population in an outpatient setting.
- Practice within an innovative, value-based care model focused on improving patient outcomes.
- Work collaboratively with a multidisciplinary healthcare team to address complex medical needs.

Duties & Responsibilities
- Conduct thorough patient assessments, diagnose conditions, and develop individualized care plans.
- Manage chronic illnesses common among seniors, such as heart disease, diabetes, and hypertension.
- Provide outpatient care, including preventive health screenings, disease management, and health education.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Utilize electronic health records and maintain accurate documentation of patient encounters.
- Collaborate with other providers, specialists, and care coordinators to optimize patient outcomes.
- Engage patients and families in shared decision-making regarding treatment options and health goals.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and innovation.
- Support the organization’s mission by participating in wellness activities and community outreach as appropriate.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license and current DEA registration.
- Proficient in outpatient office procedures, including suturing, joint injections, skin biopsies, and wound care.
- Excellent communication, interpersonal, and clinical skills.
- Fellowship in Geriatrics is a plus but not required.
- Demonstrated commitment to value-based care and patient-centered practice.
