Primary Care Physician (MD/DO) - Summary- Deliver comprehensive primary care to a predominantly senior patient population in an outpatient setting.
- Practice within an innovative, value-based care model focused on improving patient outcomes.
- Work collaboratively with a multidisciplinary healthcare team to address complex medical needs.
- Foster strong, ongoing relationships with patients and their families to ensure continuity of care.
- Promote a culture of respect, empathy, and excellence in patient care.
- Conduct thorough patient assessments, diagnose conditions, and develop individualized care plans.
- Manage chronic illnesses common among seniors, such as heart disease, diabetes, and hypertension.
- Provide outpatient care, including preventive health screenings, disease management, and health education.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Utilize electronic health records and maintain accurate documentation of patient encounters.
- Collaborate with other providers, specialists, and care coordinators to optimize patient outcomes.
- Engage patients and families in shared decision-making regarding treatment options and health goals.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and innovation.

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license and current DEA registration.
- Proficient in outpatient office procedures, including suturing, joint injections, skin biopsies, and wound care.
- Excellent communication, interpersonal, and clinical skills.
- Fellowship in Geriatrics is a plus but not required.
- Demonstrated commitment to value-based care and patient-centered practice.
- Experienced in managing patients with complex chronic conditions, particularly seniors.
- Knowledgeable in value-based care principles and dedicated to improving patient outcomes.
- Collaborative, compassionate, and eager to work as part of a multidisciplinary team.
- Adaptable to new care models and open to integrating technology in daily practice.
- Motivated to contribute to a positive, inclusive, and supportive clinic environment.
