This full-time Primary Care Physician (MD/DO) position offers you the opportunity to practice medicine in a manner that emphasizes quality over quantity, with an intentionally reduced patient panel and the time to address each patient's unique health needs. You will work alongside a dedicated care team, including a medical assistant, scheduler, and referral specialist, allowing you to focus on patient care while administrative support streamlines your workflow.

Primary Care Physician (MD/DO) – Summary
- Provide comprehensive outpatient primary care for a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Collaborate with a multidisciplinary care team to coordinate patient care and improve health outcomes.

Duties & Responsibilities
- Conduct routine and preventive primary care exams for adult and senior patients in an outpatient clinic setting.
- Diagnose and manage chronic illnesses such as diabetes, hypertension, cardiovascular disease, and other age-related conditions.
- Provide ongoing care management, medication adjustments, and coordination of specialty referrals as needed.
- Work with a reduced patient panel, seeing 18-22 patients per day, allowing for higher quality, individualized care.
- Participate in daily clinic operations, including scheduled administrative time for charting, care coordination, and patient follow-up.
- Collaborate with consulting specialists in fields such as cardiology and podiatry, as well as holistic health providers offering acupuncture, tai chi, yoga, and nutrition services.
- Engage in value-based initiatives to drive superior patient outcomes and contribute to the health system's success.
- Ensure all care provided meets the highest clinical standards and complies with applicable medical guidelines and regulations.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of a residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine.
- Ability to communicate effectively and work collaboratively in a multidisciplinary care environment.
- Spanish language proficiency is preferred but not required.
