This role centers around delivering comprehensive primary care to a small, manageable panel of patients, primarily seniors with multiple chronic conditions. You will be supported by a dedicated care team, including medical assistants, schedulers, and referral specialists, ensuring that you have the resources needed to provide top-level care. The organization’s unique model allows for in-house consulting specialists, holistic health services, and even door-to-door transportation for patients, removing barriers to accessing care.
With a Monday through Friday schedule, no nights or weekends, and administrative time built into your day, this position fosters a sustainable work-life balance. Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Deliver comprehensive, compassionate primary care to a panel of up to 450 patients, primarily seniors with multiple chronic diseases
- Maintain a patient schedule of up to 18 patients per day, Monday through Friday, with hours from 7:30 AM to 5:00 PM (last patient scheduled at 4:15 PM)
- Utilize value-based care principles to improve patient outcomes and satisfaction
- Coordinate care with dedicated clinical support staff, including a medical assistant, scheduler, and referral specialist
- Collaborate with in-house consulting specialists (Cardiology, Podiatry, and more) as needed
- Participate in administrative tasks, with designated time allocated in the daily schedule
- Engage in holistic health initiatives, including referring patients to services such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Contribute to organizational mission and values, upholding high standards of patient dignity and respect

Qualifications & Requirements
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine
- Commitment to value-based care and improving outcomes for seniors with complex health needs
- Spanish language proficiency preferred, but not required
- Excellent interpersonal, communication, and teamwork skills
