This role is ideal for a board-certified physician who is passionate about delivering high-quality, patient-centered care to individuals aged 65 and older, particularly those enrolled in Medicare Advantage plans. The position is available immediately and offers the potential for ongoing extension, with a minimum commitment of three months and the possibility of transitioning to a permanent role. The Primary Care Physician (MD/DO) will work closely with a robust support staff to manage a manageable patient census, focusing on comprehensive care and the reduction of emergency room visits and hospital admissions.
This contract-based position provides a structured Monday through Friday schedule from 8 am to 5 pm, allowing for work-life balance while making a meaningful impact in the community. The outpatient clinical setting is supported by a full staff, ensuring that the Primary Care Physician (MD/DO) can focus on direct patient care without administrative overload. The clinical approach is specifically designed to address the health needs of seniors, emphasizing preventive care and chronic disease management to enhance quality of life and reduce unnecessary healthcare utilization.
Physicians with experience in Medicare Advantage and a commitment to evidence-based, compassionate care will thrive in this environment. The facility offers expedited credentialing for qualified candidates, aiming for an ASAP start to fill this critical role. Travel expenses are covered, and all necessary support is provided to ensure a smooth onboarding process. The position requires an active state medical license, an active or transferable DEA license, and current BLS certification. Board certification in Internal Medicine (ABIM) or Family Medicine (ABFM) is mandatory, with a preference for those experienced in senior care and familiar with the nuances of outpatient geriatric medicine.
The Primary Care Physician (MD/DO) will be responsible for managing a patient panel of approximately 12-18 seniors per day, delivering all standard primary care services. The clinical model is designed to prioritize continuity of care, proactive health maintenance, and early intervention to prevent complications. This includes routine check-ups, chronic disease management, medication management, coordination with specialists, and comprehensive patient education. The role also involves close collaboration with interdisciplinary team members to ensure holistic care for every patient.
