Primary Care Physician (MD/DO) - Summary
- Deliver world-class, coordinated primary care to moderate- to low-income seniors with multiple chronic diseases
- Work within a collaborative, physician-led team environment

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, focusing on seniors with 3-5 chronic diseases
- Conduct patient visits in an outpatient setting (approximately 18 patients per day)
- Coordinate care with a dedicated team including a medical assistant, scheduler, and referral specialist
- Participate in value-based care initiatives aimed at improving patient outcomes and reducing healthcare costs
- Utilize scheduled administrative time for charting, care coordination, and professional collaboration
- Refer patients to in-house consulting specialists when needed, including cardiology and podiatry
- Engage in holistic, patient-centered programs such as acupuncture, tai chi, yoga, and nutrition services
- Maintain compliance with all relevant clinical and regulatory standards
- Foster long-term relationships with patients to support their ongoing health needs

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in FM, IM, or Geriatric Medicine
