Primary Care Physician (MD/DO) - Summary
- Provide comprehensive primary care to a senior patient population in a supportive, team-based environment.
- Utilize a value-based care model focused on improving patient outcomes and satisfaction.

- Participate in a collaborative, multidisciplinary approach to healthcare delivery.
Duties & Responsibilities
- Deliver outpatient primary care for adults, primarily seniors with multiple chronic conditions.
- Conduct thorough assessments, develop and implement care plans, and manage ongoing treatment for chronic diseases such as heart disease, diabetes, and hypertension.
- Work alongside a dedicated care team, including medical assistants, schedulers, and referral specialists.
- Coordinate with leadership and utilize available resources to optimize patient care and access.
- Participate in value-based initiatives, focusing on patient satisfaction, adherence to clinical quality metrics (e.g., HEDIS compliance), and reduction of hospitalizations.
- Utilize electronic health record (EHR) systems to document patient care activities and communicate effectively with the healthcare team.
- Engage in community and clinic-based wellness activities designed to support holistic patient health.
- Uphold the highest standards of professionalism, ethical conduct, and patient confidentiality at all times.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license and current DEA registration.
- Proficiency in office procedures, such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship in Geriatrics is an advantage but not mandatory.
- Strong computer skills and familiarity with EHR systems.
- Excellent interpersonal and communication skills, with a commitment to inclusive, patient-centered care.
