Primary Care Physician (MD/DO) - Summary
- Deliver outpatient primary care services to seniors with multiple chronic conditions
- Practice within a physician-led, value-based care model
- Focus on relationship-building and personalized patient support
- Work with a reduced patient panel for enhanced care quality
Duties & Responsibilities
- Provide comprehensive primary care to a panel of approximately 450 patients, typically seeing 18–22 patients per day
- Conduct thorough health assessments, manage chronic diseases, and develop tailored care plans for each patient
- Utilize value-based care strategies to improve outcomes and patient satisfaction
- Collaborate with in-house consulting specialists, including cardiology and podiatry, to ensure continuity of care
- Incorporate holistic health services into patient care, such as acupuncture, tai chi, yoga, and nutritional counseling
- Work closely with a dedicated support team, including a Medical Assistant, Scheduler, and Referral Specialist
- Participate in administrative activities and care coordination during scheduled work hours
- Maintain up-to-date and accurate medical records using electronic health record systems
- Adhere to all regulatory and organizational standards for quality and compliance
- Promote patient engagement, education, and self-management

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training completed in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Bilingual in English and Spanish preferred to better serve patient population
- Demonstrated commitment to compassionate, patient-centered care
- Experience managing patients with multiple chronic conditions is highly valued
- Strong communication, organizational, and interpersonal skills
- Ability to work effectively in a team-based care model
