As a Primary Care Physician (MD/DO), you will play a pivotal role in transforming the healthcare experience for a population with complex chronic conditions. You will be supported by a dedicated care team—including your own Medical Assistant, Scheduler, and Referral Specialist—enabling you to focus on patient care and clinical leadership. The clinic operates on a Monday through Friday schedule, promoting work-life balance and providing administrative time within your workday. With a manageable patient panel and no hospital call or night shifts, you will have the time and resources to deliver comprehensive and personalized care.
The patient population primarily comprises seniors managing multiple chronic diseases, and the practice is committed to a value-based care model. The clinic environment fosters holistic health, featuring access to consulting specialists such as cardiology and podiatry, as well as complementary wellness services like acupuncture, Tai Chi, yoga, and nutrition counseling. Patients benefit from door-to-door transportation at no charge, further supporting access to care.

Primary Care Physician (MD/DO) - Summary- Physician-led, mission-driven organization focused on senior primary care
- Full-time role delivering value-based, comprehensive outpatient care
- Collaborative environment with dedicated clinical support staff
- Provide high-quality, evidence-based primary care to a panel of up to 450 senior patients with multiple chronic conditions
- Conduct patient visits (18-22 per day) within a Monday-Friday clinic schedule
- Coordinate care with in-house consulting specialists and holistic health providers
- Utilize dedicated administrative time for care planning and patient follow-up
- Participate in value-based care initiatives and continuous quality improvement
- Work closely with your care team, including Medical Assistant, Scheduler, and Referral Specialist
- Leverage clinic resources such as transportation services to enhance patient access
- Maintain thorough and accurate patient documentation

- Medical degree (MD or DO) from an accredited institution
- Completion of residency in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Bilingual Spanish-speaking skills preferred
