Joining our team as a Primary Care Physician (MD/DO) means you will work with a small patient panel, allowing for a personalized approach to medicine and the ability to foster strong patient-provider relationships. Patients benefit from a variety of in-house services, including consulting specialists and holistic health offerings, while providers are supported by a dedicated care team, including a medical assistant, scheduler, and referral specialist. Our senior patient population often presents with multiple chronic conditions, providing an impactful and challenging environment for clinical growth.
Our clinic is designed to support both patient and provider well-being. Providers enjoy a Monday to Friday schedule with no night shifts or in-hospital call responsibilities, ensuring ample time for family and personal pursuits. Administrative time is built into your schedule, supporting optimal workflow and reducing burnout. Primary Care Physician (MD/DO) - Summary
- Full-time outpatient position for a board-certified or board-eligible Internal Medicine, Family Medicine, or Geriatric Medicine physician
- Serve a senior patient population with high-touch, value-based care
- Collaborative, physician-led team environment
Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic diseases
- See an average of 18-22 patients per day in a fully outpatient setting
- Work closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Engage in value-based care initiatives to improve patient outcomes
- Coordinate care with in-house consulting specialists, including cardiology and podiatry
- Utilize administrative time included in your schedule for patient follow-up and charting
- Participate in clinic-offered holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Uphold the clinic’s mission to deliver affordable, high-quality care for seniors

Qualifications & Requirements
- Medical degree (MD or DO) and completion of a residency in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board certification or eligibility in relevant specialty (IM, FM, or Geriatrics)
- Commitment to value-based primary care delivery
- Strong communication and interpersonal skills
- Dedication to serving senior patient populations and those with complex healthcare needs

- Clinic hours are Monday through Friday, 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
