Our organization operates a network of neighborhood primary care centers designed to improve access and outcomes for the local Medicare community. By utilizing a value-based care model, you will work alongside other physicians, nurse practitioners, medical assistants, scribes, and social workers in a modern, team-oriented environment. You will benefit from onsite laboratory facilities, specialist consultation access, and comprehensive support staff, all focused on minimizing hospital admissions and maximizing preventive care.
As a Primary Care Physician (MD/DO), your patient panel will consist of 13-16 daily visits, allowing you adequate time to provide thorough, patient-centered care. Regular appointments are scheduled for 20 minutes, with new and complex diagnostic visits allocated 40 minutes, ensuring you can address patient needs effectively. The care team model provides a scribe and medical assistant for documentation support and a social worker to address social determinants of health, enabling you to devote your attention to clinical decision-making and patient relationship building.
Our centers are equipped with onsite labs staffed by phlebotomists and provide seamless access to specialist opinions through RubiconMD. The electronic medical record systems, Canopy and Greenway, streamline clinical workflow and support coordinated care. Community engagement is a core value, and patients enjoy an array of wellness and social activities, including exercise classes, events, yoga, dance, and spa days in our dedicated community rooms.
Physicians at our locations typically work alongside 3-5 providers per clinic, fostering a collegial, supportive work culture.
