We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a value-based healthcare network serving Medicare patient populations. As a Primary Care Physician (MD/DO), you will benefit from a well-structured schedule, manageable patient volumes, and access to modern medical technology. You will work alongside a dedicated care team, including scribes, medical assistants, and social workers, allowing you to focus on comprehensive patient care. Our on-site laboratory, access to specialist consultations via RubiconMD, and electronic medical records (EMR) systems such as Canopy and Greenway enhance the efficiency of your daily practice. The role is ideal for physicians passionate about value-based health models and interested in an environment that rewards outcomes and quality over volume.
Joining our team as a Primary Care Physician (MD/DO) means being part of a supportive community of 3-5 providers per clinic, comprised of both nurse practitioners and physicians. The clinic offers a range of amenities, including transportation services for patients within five miles, and a community room offering wellness activities like exercise, yoga, dance, and more. Primary Care Physician (MD/DO) - Summary- Full-time position serving Medicare patient populations
- Focus on value-based, patient-centered primary care
- Collaborative, team-based approach with robust support staff

- Provide comprehensive primary care for adult and geriatric patient populations
- Conduct patient visits, including 20-minute regular appointments and 40-minute diagnostic/new patient intakes
- Manage patient panels of 13-16 appointments per day in a structured, value-based model
- Collaborate with care team members, including scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy and Greenway) for documentation and care coordination
- Access on-site laboratory services and specialist consultations via RubiconMD
- Participate in community-based wellness and engagement programs
- Focus on preventive medicine, chronic disease management, and outcome-driven care

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- No current residents; experienced physicians preferred
- Fellowship training in Geriatrics preferred but not required
- Demonstrated commitment to value-based, patient-centered care
- Strong interpersonal and communication skills for effective patient relationships
- Experience working in team-based healthcare settings
