As a Primary Care Physician (MD/DO), you will be supported by a large care team, including medical assistants, scribes, and social workers, allowing you to focus on direct patient care. Primary Care Physician (MD/DO) - Summary
- Provide high-quality, value-based primary care to Medicare patient populations in a collaborative environment.
- Focus on preventive medicine, patient outcomes, and exceptional patient experiences.
- Work closely with a large care team including medical assistants, scribes, and social workers.
- Utilize advanced EMR systems and have access to on-site labs and specialist consultations.
Duties & Responsibilities
- Deliver comprehensive primary care services to a diverse patient panel.
- Conduct regular and full diagnostic visits, allocating appropriate time for thorough assessments.
- Collaborate with care teams to coordinate patient care, ensuring continuity and follow-up.
- Utilize value-based care principles to manage patient outcomes and minimize hospital admissions.
- Engage in preventive health measures and patient education.
- Participate in community health activities and patient engagement events.
- Document patient encounters accurately in EMR systems (Canopy, Greenway).
- Leverage specialist access for complex case consultation via RubiconMD.
- Contribute to a positive and inclusive clinic culture, supporting peers and staff.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Fellowship training in Geriatrics is preferred but not required.
- Residents are not eligible for this position.
