Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care to Medicare patient populations.
- Work collaboratively with multidisciplinary care teams including scribes, medical assistants, and social workers.
- Utilize advanced EMR systems and on-site laboratory resources.
- Focus on preventive care, chronic disease management, and enhanced patient outcomes.
- Participate in community engagement and patient wellness activities.
Duties & Responsibilities
- Provide primary care services, conducting regular and diagnostic patient visits.
- Manage a patient panel with an average daily volume of 13-16 patients, allowing for individualized attention and extended visit times.
- Work Monday through Friday within standard business hours, ensuring work-life balance.
- Collaborate with care teams to coordinate patient care, reduce hospital admissions, and optimize health outcomes.
- Utilize EMR platforms (such as Canopy and Greenway) for documentation and care coordination.
- Access specialty consultations via telehealth platforms for complex cases.
- Participate in practice improvement initiatives and clinical quality programs.
- Lead and engage in patient education, preventive health programs, and wellness activities.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited medical school.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- No residents; must have completed all required training.
- Fellowship training in Geriatrics preferred, but not required.
- Bilingual proficiency in Spanish highly valued.
