Primary Care Physician (MD/DO) - Summary
- Deliver value-based primary healthcare to Medicare populations
- Focus on patient outcomes and preventive care in a collaborative team setting
- Utilize advanced technology and on-site resources for optimal patient management
- Enjoy a balanced schedule and dedicated patient support services
Duties & Responsibilities
- Provide comprehensive primary care services, including preventive, diagnostic, and therapeutic care
- Manage a patient panel of 13-16 patients per day, ensuring high-quality, personalized care
- Conduct regular visits (20 minutes) and full diagnostic/new patient visits (40 minutes)
- Collaborate with an interdisciplinary care team, including scribes, medical assistants, and social workers
- Utilize electronic medical record (EMR) systems such as Canopy and Greenway
- Participate in value-based care initiatives to enhance patient outcomes and reduce hospital admissions
- Engage in patient education, preventive medicine, and chronic disease management
- Support community health through on-site activities and patient transportation services
- Work alongside 3-5 providers per clinic, including nurse practitioners and fellow primary care physicians
- Ensure compliance with organizational policies and all relevant medical regulations

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board certified or board eligible in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Fellowship training in Geriatrics preferred, but not required
- Demonstrated commitment to patient-centered, value-based care
- No current residents will be considered for this opportunity
