Primary Care Physician (MD/DO) - Summary- Full-time opportunity in a value-based care setting focused on Medicare populations
- Patient-centered approach with a lower daily patient volume for quality care

- Collaborate with interdisciplinary care teams including scribes, medical assistants, and social workers
- Utilize onsite labs and specialty consult platforms for coordinated patient care
- Participate in value-based care initiatives to improve patient outcomes
- Manage a daily patient volume of 13-16, ensuring personalized attention
- Document patient encounters accurately in EMR (Canopy and Greenway)
- Engage in community outreach and patient activities within the clinic

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Experience in primary care or equivalent setting preferred
- Fellowship training in Geriatrics is a plus, but not required
- No current residents; experienced practitioners encouraged to apply
- Demonstrates a strong commitment to delivering high-quality, patient-centered care
- Thrives in a value-based care model focused on clinical outcomes
- Enjoys working collaboratively within multidisciplinary care teams
- Proficient in the use of EMR systems and technology-enabled care coordination
- Has a clear, unrestricted medical license and is board certified or eligible
- Interest in working with Medicare populations and older adult patients

- Transportation support for patients within a 5-mile radius of the clinic
- Average of 3-5 providers (mix of NPs and primary care physicians) per clinic
