The Primary Care Physician (MD/DO) will work in a collaborative, multidisciplinary environment, supported by medical assistants, scribes, and social workers to streamline care delivery. Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Deliver comprehensive primary care services to Medicare patients
- Focus on preventive medicine, chronic disease management, and patient education
- Collaborate with interdisciplinary care teams including scribes, medical assistants, and social workers
- Utilize onsite labs and specialty consult platforms for coordinated patient care
- Participate in value-based care initiatives to improve patient outcomes
- Manage a daily patient volume of 13-16, ensuring personalized attention
- Document patient encounters accurately in EMR (Canopy and Greenway)
- Engage in community outreach and patient activities within the clinic

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Experience in primary care or equivalent setting preferred
- Fellowship training in Geriatrics is a plus, but not required
- No current residents; experienced practitioners encouraged to apply
