Primary Care Physician (MD/DO) - Summary
- Deliver value-based primary care to Medicare patient populations in a supportive, team-based setting.
- Work with a dedicated team including scribes, medical assistants, and social workers to optimize patient outcomes.
- Utilize on-site laboratory services and access specialist consultations to streamline patient care.
- Focus on preventive medicine, quality metrics, and patient satisfaction through longer appointment times and a lower daily patient volume.

Duties & Responsibilities
- Conduct comprehensive primary care visits, including routine, diagnostic, and new patient assessments.
- Manage an average daily patient volume of 13-16 individuals.
- Collaborate with clinic staff to coordinate care and ensure a seamless patient experience.
- Leverage electronic medical record (EMR) systems such as Canopy and Greenway to maintain accurate patient documentation.
- Participate in community engagement activities and patient wellness programs hosted at the clinic.
- Promote preventive care and chronic disease management for optimal patient health outcomes.
- Support transportation and community activities to enhance patient access and engagement.
- Contribute to a collaborative, multidisciplinary clinic environment that prioritizes patient-centered care.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license.
- Fellowship training in Geriatrics preferred but not required.
- No current residents.
