The practice is equipped with on-site laboratory facilities, including a phlebotomist, and offers access to a network of specialists through RubiconMD for streamlined referrals. Using state-of-the-art EMR systems such as Canopy and Greenway, you will be empowered to deliver efficient and coordinated care for every patient.
The value-based care model is designed to reduce unnecessary hospital admissions and prioritize preventive medicine, ensuring every patient receives consistent, high-quality support. Primary Care Physician (MD/DO) – Summary- Deliver value-based, patient-centered primary care to a local Medicare population
- Work in a supportive, team-based environment with comprehensive resources
- Focus on quality outcomes, preventive medicine, and patient satisfaction
- Utilize advanced EMR systems and on-site laboratory facilities
- Provide comprehensive primary care to adult and senior patients, focusing on preventive health and chronic disease management
- Manage a daily patient volume of 13-16, ensuring adequate time for thorough assessments
- Conduct regular 20-minute visits and 40-minute diagnostic or new patient evaluations
- Collaborate with a multidisciplinary care team, including scribes, medical assistants, and social workers
- Utilize on-site laboratory services and access to specialists via RubiconMD as needed
- Document all patient interactions accurately using Canopy and Greenway EMRs
- Participate in value-based care initiatives to reduce unnecessary hospitalizations and improve patient outcomes
- Engage in community wellness activities and support holistic patient health through educational events
- Competitive base salary ($230,000–$250,000) with incentive bonus based on quality metrics

- Graduate of an accredited school of medicine (MD or DO)
- Board certified or board eligible in Internal Medicine or Family Medicine
- Active, non-probationary, unrestricted state medical license
- Bilingual in Spanish preferred
- Fellowship training in Geriatrics preferred, but not required
- Not accepting residents at this time
- Experienced Primary Care Physician (MD/DO) with a passion for value-based, patient-centered care

- Bilingual in Spanish is a plus
