Primary Care Physician (MD/DO) - Summary
- Provide comprehensive, outcome-based primary care to Medicare patients
- Full-time position in a collaborative, innovative clinical setting
- Focus on preventive medicine and minimizing hospital admissions
- Supported by a multidisciplinary care team
- Utilize modern EMR and onsite lab facilities
Duties & Responsibilities
- Deliver primary care to a panel of 13-16 patients per day, including regular and full diagnostic visits
- Participate in value-based care initiatives aimed at measurable quality outcomes
- Collaborate with care team members, including scribes, medical assistants, and social workers, to ensure holistic patient management
- Utilize EMR systems (Canopy, Greenway) for accurate documentation and care coordination
- Access specialist consultations through RubiconMD as needed
- Engage with patients in community room activities focused on health and wellness
- Contribute to ongoing quality improvement and patient satisfaction efforts
- Maintain compliance with all clinical policies, procedures, and state regulations

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree
- Board Certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted medical license
- Bilingual proficiency in Spanish preferred
- No resident physicians; experienced candidates strongly encouraged
- Fellowship training in Geriatrics is a plus, but not required
