Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive, patient-focused primary care to Medicare populations using a value-based healthcare model.
- Collaborate with a multidisciplinary care team including scribes, medical assistants, and social workers.
- Utilize onsite labs, EMR systems (Canopy and Greenway), and access specialist consultations via RubiconMD.
- Support and foster patient engagement through community room activities and transportation assistance for patients.
- Work within a well-staffed clinic environment, typically with 3–5 providers per site.
Duties & Responsibilities
- Provide primary medical care and preventive services to Medicare patient populations.
- Conduct thorough patient assessments, diagnostics, and ongoing wellness visits (average 13–16 patients per day).
- Document all patient interactions and clinical notes accurately using EMR systems (Canopy and Greenway).
- Work collaboratively with the care team to coordinate personalized care plans and ensure high-quality outcomes.
- Participate in the development and execution of value-based care strategies to reduce hospital admissions and improve patient satisfaction.
- Engage patients in wellness activities including exercise, events, yoga, dance, and other community-based programs.
- Attend to both regular and new patient visits, allocating appropriate time for diagnostics (20–40 minutes per visit).
- Contribute to team-based care meetings and quality improvement initiatives as part of the practice’s mission.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, non-probationary state medical license.
- Spanish language proficiency preferred to serve the local population effectively.
- Fellowship training in Geriatrics is preferred but not required.
- New residents are not considered at this time.
