We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative network of neighborhood primary care centers in Chicago, IL. Our care model is designed to reward positive patient outcomes, minimize unnecessary hospital admissions, and promote preventive medicine. You will be supported by a team that includes scribes, medical assistants, and social workers, allowing you to dedicate more time to direct patient care and less to administrative tasks.
Our clinics are equipped with on-site laboratories, phlebotomy services, and advanced electronic medical record (EMR) systems such as Canopy and Greenway. You will also have access to specialist consultations through RubiconMD, and patients benefit from convenient transportation options within five miles of the clinic. Community engagement is encouraged with a variety of patient activities, including exercise classes, events, yoga, dance, and wellness programs.
As a valued member of our team, you will work alongside 3-5 other healthcare providers, including Nurse Practitioners and fellow Primary Care Physicians, to deliver comprehensive care in a supportive and dynamic setting. Primary Care Physician (MD/DO) - Summary
- Provide comprehensive primary care to Medicare patient populations
- Deliver value-based, outcome-driven healthcare in a supportive, collaborative environment
- Work full-time in a modern neighborhood clinic equipped with on-site labs and EMR systems
- Join a multidisciplinary care team with access to specialist consultation and patient support programs
Duties & Responsibilities
- Conduct routine and diagnostic patient visits, averaging 13-16 patients per day
- Participate in a value-based care model focused on preventive medicine and low hospital admission rates
- Work closely with a care team including scribes, medical assistants, and social workers
- Utilize EMR systems (Canopy, Greenway) for efficient patient record management
- Access to on-site labs and phlebotomy services, as well as RubiconMD for specialist input
- Engage patients in community activities and wellness programs
- Coordinate with 3-5 providers per clinic, fostering a collaborative, team-based approach

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license (non-probationary)
- Bilingual Spanish-speaking ability required
- Fellowship training in Geriatrics preferred, but not required
- Residents are not eligible for this role
