The ideal candidate will be board-certified or board-eligible in Internal Medicine or Family Medicine, with an active, unrestricted medical license. Bilingual Spanish-speaking skills are highly valued, and fellowship training in geriatrics is preferred but not required. You will work with modern electronic medical record (EMR) systems such as Canopy and Greenway and benefit from on-site lab facilities, ready access to specialty care consultations, and a comprehensive suite of patient-focused programs and activities.

Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care to Medicare patient populations as a full-time Primary Care Physician (MD/DO).
- Join a collaborative care team with access to advanced EMR systems and on-site lab resources.
- Make a lasting impact on patient health outcomes while working in a dynamic and supportive environment.
Duties & Responsibilities
- Provide direct patient care, focusing on comprehensive assessments, preventive medicine, and chronic disease management.
- Collaborate with a multidisciplinary care team, including medical assistants, scribes, and social workers, to ensure coordinated and effective care delivery.
- Utilize value-based care principles to optimize patient outcomes and reduce unnecessary hospital admissions.
- Manage a patient volume of approximately 13-16 patients per day, allowing for meaningful patient interactions and thorough care.
- Conduct patient visits with sufficient time for regular appointments (20 minutes) and extended diagnostic or new patient visits (40 minutes).
- Leverage advanced EMR platforms (Canopy and Greenway) for accurate documentation and care coordination.
- Engage patients in community wellness programs and activities, such as exercise classes, educational events, and wellness support.
- Partner with the care team to facilitate patient transportation services and access to specialty consultations through platforms such as RubiconMD.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Bilingual proficiency in Spanish is strongly preferred.
- Fellowship training in Geriatrics is a plus but not required.
- Current residents are not eligible for this position.
