Physicians with experience in value-based care (VBC) are encouraged to apply, though comprehensive onboarding and training are available for those new to the model.
In this role, you will manage a panel of Medicare Advantage patients, focusing on both chronic disease management and preventive health. Primary Care Physician (MD/DO) - Summary- Full-time Primary Care Physician role focused on adults 65+ in a value-based care environment
- Located in Corpus Christi, TX, with a supportive, mission-driven team
- Emphasis on preventive care, chronic disease management, and quality patient interactions
- Provide comprehensive primary care to a panel of Medicare Advantage patients, focusing on prevention and chronic disease management
- See approximately 12–15 patients per day, allowing for extended visit times (20–40 minutes per patient)
- Collaborate with an integrated care team including NP/PA, medical assistants, social workers, pharmacy, and lab staff
- Participate in care coordination, referral management, and interdisciplinary meetings
- Utilize an electronic medical record (EMR) system and follow documentation best practices
- Engage in ongoing training and professional development, including a comprehensive onboarding program

- Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Experience with geriatric patients preferred
