Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive, value-based primary care to a diverse Medicare patient population.
- Work collaboratively within a multidisciplinary care team to optimize patient outcomes.
- Utilize innovative care models to prioritize prevention and reduce unnecessary hospitalizations.
- Access on-site lab and specialist consultation resources.
Duties & Responsibilities
- Provide primary care services to an average of 13-16 patients daily.
- Conduct patient visits averaging 20 minutes for established patients and 40 minutes for new or complex cases.
- Lead a care team composed of scribes, medical assistants, and social workers.
- Utilize electronic medical records (Canopy and Greenway) for accurate documentation.
- Participate in a value-based healthcare model focused on quality and preventive care.
- Engage patients in wellness and community activities, such as exercise and educational events.
- Coordinate with on-site labs and access specialist consults as necessary.
- Ensure a patient-centered experience, including supporting transportation for qualifying patients.

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- No current residency status; this role is for fully licensed physicians.
- Fellowship training in Geriatrics is preferred but not required.
