Primary Care Physician (MD/DO) - Summary
The Primary Care Physician (MD/DO) will provide direct medical care to an adult and geriatric patient panel, utilizing a value-based care approach in an outpatient setting. You will work closely with a multi-disciplinary team, supported by experienced staff including medical assistants, nurses, case managers, and outreach specialists. Your focus will be on preventive care, chronic disease management, and ensuring the best possible quality of life for your patients. The role is designed for a dedicated physician who values continuity of care, collaboration, and evidence-based practice.
Duties & Responsibilities
- Deliver comprehensive primary care to patients aged 65 and older in an outpatient facility using a value-based care model.
- Conduct patient visits during weekday shifts, Monday through Friday, from 8:00 AM to 5:00 PM.
- Manage a patient panel with an average of 10-13 patients per day, with a maximum panel of 400 patients.
- Collaborate with an integrated care team that includes a scheduler, medical assistant (MA), referral specialist, nurse, case manager, social worker, and outreach specialist.
- Provide preventive care, chronic disease management, wellness exams, and patient education to optimize health outcomes.
- Order and interpret diagnostic tests, including EKGs and laboratory studies performed onsite.
- Utilize the Epic electronic medical record (EMR) system to document visits, maintain patient records, and coordinate care.
- Participate in care team meetings and contribute to quality improvement initiatives.
- Ensure compliance with all applicable healthcare regulations and organizational policies.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree, with board certification in Internal Medicine (IM) or Family Medicine (FM).
- Active and unrestricted state medical license.
- Valid state DEA registration.
- Proficiency in using electronic medical record systems, preferably Epic.
- Demonstrated commitment to value-based care and patient-centered practice.
- Strong communication, organizational, and interpersonal skills.
- Ability to work collaboratively within a multidisciplinary team.
