Primary Care Physician (MD/DO) - Full-Time Opportunity

We are seeking a dedicated and experienced Primary Care Physician (MD/DO) to join our team and provide comprehensive outpatient care in a value-based care facility. This full-time position offers the chance to make a meaningful impact on the health and wellness of a predominantly senior patient population while working within a collaborative and supportive clinical environment. As a Primary Care Physician (MD/DO), you will utilize your training in Internal Medicine (IM) or Family Medicine (FM) to deliver high-quality healthcare services to adult patients, primarily those aged 65 and above. You will be responsible for managing a patient panel, conducting thorough medical evaluations, and collaborating closely with a multidisciplinary care team to support holistic patient outcomes. The facility is equipped with on-site resources including EKGs and laboratory services, ensuring a seamless workflow and comprehensive care experience for both providers and patients. The use of the Epic electronic medical record (EMR) system streamlines documentation and care coordination, supporting your ability to focus on clinical excellence.

Our facility operates Monday through Friday from 8am to 5pm, allowing for a predictable work schedule that supports work-life balance. Each physician is supported by their own dedicated care team, including a scheduler, medical assistant (MA), referral specialist, nurse, case manager, social worker, and outreach specialist. This structure ensures you have the administrative and clinical support necessary to provide exceptional care to a manageable patient load, with an average of 10-13 patients per day and a maximum patient panel size of 400.

The ideal candidate for this Primary Care Physician (MD/DO) role will hold an active medical license, a valid DEA registration, and board certification. Physicians with a passion for value-based care and a commitment to improving patient outcomes for older adults are encouraged to apply. The credentialing process is efficient, typically completed within two to three weeks.
