As a Primary Care Physician (MD/DO) in our adult outpatient setting, you will deliver comprehensive care for a Medicare Advantage patient population, with a focus on chronic disease management and preventive health services. You will have the support of a highly collaborative, multidisciplinary team designed to enable you to practice medicine without the administrative burden and pressure often found in traditional fee-for-service environments. Our model empowers you to provide high-quality care with manageable patient volumes and extended visit lengths, allowing for deeper patient engagement and impactful clinical interventions.

Primary Care Physician (MD/DO) - Summary
- Full-time outpatient primary care role focused on adults 65+ (majority Medicare Advantage)
- Value-based care model emphasizing preventive services and chronic disease management

Duties & Responsibilities
- Provide high-quality primary care to a panel of adult patients aged 65 and above
- Conduct thorough patient assessments and develop personalized care plans
- Focus on prevention, early intervention, and chronic disease management in alignment with value-based care principles
- Spend 20-40 minutes per patient visit, allowing for in-depth engagement and relationship-building
- Maintain a daily census of 12-15 patients to support comprehensive care delivery
- Collaborate with nurse practitioners, physician assistants, medical assistants, social workers, lab, pharmacy, and referral coordinators to provide integrated care
- Participate in team meetings, case reviews, and quality improvement initiatives
- Utilize electronic medical records (EMR) and document care in accordance with best practices
- Engage in ongoing professional development and training, including a robust onboarding program

Qualifications & Requirements
- MD or DO degree with Board Certification or Board Eligibility in Internal Medicine or Family Medicine
- Experience in geriatric care is preferred but not required
- Experience with value-based care or HEDIS quality measures is a plus
- Eligibility for medical licensure
- Strong commitment to providing patient-centered, quality-driven care
