Primary Care Physician (MD/DO) – Full-time

The Primary Care Physician (MD/DO) role provides you with the framework and resources necessary to succeed in a mission-driven environment, including comprehensive onboarding, ongoing training, and an emphasis on professional development.
As a Primary Care Physician (MD/DO), you will care for a patient population primarily composed of adults 65 and over, focusing on chronic disease management and preventive care. You’ll benefit from longer visit times, averaging 20 to 40 minutes per patient, which allows you to deliver thorough, high-quality medical care. With a manageable daily census of 12 to 15 patients, you’ll have the time to address complex needs and form lasting relationships with those in your care.
This Primary Care Physician (MD/DO) position is supported by a multidisciplinary team, including Nurse Practitioners (NPs), Physician Assistants (PAs), Medical Assistants, Social Workers, and in-house lab and pharmacy support. You’ll also have access to dedicated referral coordination to streamline patient care. Our onboarding process includes a comprehensive three-week Physician Learning Journey, designed to ensure you are fully prepared to succeed in a value-based care setting. Training covers value-based care principles, documentation best practices, EMR optimization, and workflow integration within the care team. Ongoing professional development and mentorship are provided to help you continuously advance your skills.
We are looking for Primary Care Physicians (MD/DO) who are board certified or board eligible in Internal Medicine or Family Medicine. Candidates with geriatric experience or HEDIS familiarity are preferred, though not required.
