Our practice emphasizes quality over quantity, supporting physicians with an integrated care team that includes Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordinators. Your patient panel will consist primarily of Medicare Advantage patients aged 65 and above, focusing on preventive care and chronic disease management. With an average daily census of 12-15 patients and extended visit times ranging from 20 to 40 minutes, you will have the ability to give each patient the attention they deserve.

This includes extensive training in documentation, EMR optimization, and workflow within the integrated care model. Primary Care Physician (MD/DO) - Summary- Full-time role providing primary care for adults aged 65+, focused on preventive and chronic disease management

- Board Certified or Board Eligible in Internal Medicine or Family Medicine
