Primary Care Physician (MD/DO) – Full-Time

As a Primary Care Physician (MD/DO), you will have the unique opportunity to shape the health and wellness of the senior population, working within an integrated care team that includes Nurse Practitioners, Physician Assistants, Medical Assistants, Social Workers, and dedicated support in lab and pharmacy coordination. Our model allows you to devote the time and attention needed for comprehensive patient care, with an average patient census of 12-15 individuals per day and extended visit times of 20-40 minutes each. This structure supports thorough assessments, personalized care plans, and close follow-up to ensure the best possible health outcomes.
