Primary Care Physician (MD/DO) - Summary
- Full-time position focused on proactive, preventive, and chronic care for adults 65+ in a value-based care model

Duties & Responsibilities
- Provide comprehensive primary care to a predominantly Medicare Advantage patient panel, focusing on chronic disease management, preventive health screenings, and wellness assessments
- See an average of 12-15 patients per day, allowing for more individualized patient care
- Collaborate with an integrated team including Nurse Practitioners/Physician Assistants (NP/PA), Medical Assistants, Social Workers, and in-house Lab & Pharmacy support to deliver holistic patient care
- Engage actively in value-based care initiatives, including quality improvement processes and effective care coordination
- Participate in care planning meetings and contribute to the development of individualized patient care plans
- Document patient visits, care plans, and outcomes accurately and in a timely manner in the electronic health record (EHR) system
- Foster strong, collaborative relationships with patients, their families, and the care team to ensure optimal outcomes
- Refer patients to specialists and coordinate follow-up for advanced care needs as appropriate

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Geriatric experience is highly valued
- Familiarity with HEDIS measures is a plus
- Experience in Value-Based Care (VBC) preferred but not required; training will be provided for candidates eager to learn
- Current and unrestricted medical license in the United States
- Strong commitment to high-quality, compassionate patient care
- Excellent interpersonal, communication, and teamwork skills
