Primary Care Physician (MD/DO) – Summary
- Provide exceptional primary care to adults 65+ with a focus on preventive medicine and chronic disease management.
- Practice within a value-based care (VBC) model, emphasizing quality and outcomes over volume.
- Be part of a multidisciplinary team including nurse practitioners, physician assistants, medical assistants, social workers, and support staff.
- Enjoy a manageable daily patient census and longer visit times, allowing for comprehensive patient care and relationship-building.

Duties & Responsibilities
- Deliver direct primary care services to a senior patient population (primarily Medicare Advantage), ensuring high standards of preventive and chronic illness care.
- Conduct thorough patient assessments, develop individualized care plans, and work closely with the care team to coordinate comprehensive treatment.
- Maintain an average patient census of 12-15 patients per day, with 20-40 minutes allocated per visit to support thorough and meaningful patient interactions.
- Collaborate with nurse practitioners, physician assistants, social workers, medical assistants, and pharmacy and laboratory support teams to optimize patient outcomes.
- Participate in quality improvement initiatives focused on value-based care and performance metrics (including HEDIS).
- Apply current best practices in electronic medical record (EMR) documentation, with training and support provided for the organization's EMR system.
- Engage in ongoing professional development and value-based care training through structured onboarding and continuous education programs.

Qualifications & Requirements
- Medical degree (MD or DO) with Board Certification or Board Eligibility in Internal Medicine or Family Medicine.
- Experience caring for geriatric populations preferred; interest in value-based care strongly desired.
- Familiarity with HEDIS measures and quality improvement processes considered a plus.
- Strong communication, collaboration, and team-oriented skills.
