Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive and equitable primary care services to adult and senior Medicare patients
- Conduct patient visits with a focus on quality outcomes rather than volume, typically seeing 13-16 patients daily
- Perform 20-minute regular visits and 40-minute full diagnostic or new patient appointments
- Collaborate closely with a dedicated care team, including a scribe, medical assistant, and social worker
- Utilize on-site lab services and electronic medical record systems, including Canopy and Greenway
- Engage in value-based health initiatives to drive preventive care and lower hospital admissions
- Participate in community room activities, such as exercise classes, events, yoga, dance, and wellness days for patients
- Coordinate patient care with specialists via access to RubiconMD for consults
- Support patient transportation initiatives, providing van pick-up services within five miles of the clinic
- Work as part of a team of 3-5 providers per clinic, including nurse practitioners and fellow primary care physicians

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution
- Board certified or eligible in Internal Medicine or Family Medicine
- Active, unrestricted state medical license (in good standing and non-probationary)
- Preference for candidates with fellowship training in Geriatrics (not required)
- Not currently accepting residents for this role
