Primary Care Physician (MD/DO) - Summary
- Serve as a key member of a physician-led, mission-driven team focused on delivering value-based care to seniors.
- Enjoy a stable, full-time position with a Monday to Friday schedule and no night or weekend responsibilities.
- Care for a small, manageable patient panel (up to 450 patients) for more meaningful patient relationships and improved care continuity.
- Leverage a robust care team, including your own medical assistant, scheduler, and referral specialist, to enhance patient outcomes and streamline workflow.
Duties & Responsibilities
- Provide comprehensive primary care services to a predominantly senior patient population with 3-5 chronic conditions.
- Conduct patient assessments, develop individualized care plans, and coordinate ongoing care for chronic and acute conditions.
- Maintain an average daily patient volume of 18-22, enabling focused, high-quality interactions and thorough care delivery.
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) and utilize holistic health resources such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Participate in value-based care initiatives to improve health outcomes and patient satisfaction.
- Dedicate scheduled administrative time to documentation, care coordination, and professional development.
- Engage in interdisciplinary team meetings and contribute to continuous practice improvement efforts.

Qualifications & Requirements
- Medical degree (MD or DO) with completion of a residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in one of the following: Family Medicine, Internal Medicine, or Geriatric Medicine.
- Eligibility for medical licensure in Georgia.

- 100% outpatient role – no inpatient or hospital rounding required.
- No night or weekend shifts; clinic hours are Monday through Friday, 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm).
