As a Primary Care Physician (MD/DO), you will lead the care for a panel of up to 450 seniors, most of whom manage multiple chronic diseases. Our approach is rooted in value-based care, leveraging in-house consulting specialists in areas like cardiology and podiatry, as well as holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling. We strive to offer a comprehensive healthcare experience, with door-to-door patient transportation and a robust support structure for both staff and patients.

Primary Care Physician (MD/DO) - Summary
- Provide primary care to a panel of up to 450 patients, primarily seniors with multiple chronic conditions.
- Work full-time in a 100% outpatient setting with a strong focus on value-based care.
- Collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists.
- Practice medicine in a supportive, physician-led environment that prioritizes patient relationships and quality outcomes.

Duties & Responsibilities
- Conduct comprehensive primary care visits for seniors, averaging 18-22 patients per day.
- Diagnose, treat, and manage a wide range of chronic conditions common in the senior population.
- Participate in value-based care initiatives to improve patient outcomes and reduce healthcare costs.
- Coordinate care with in-house specialists and holistic health services to address all aspects of patient well-being.
- Utilize electronic health records and participate in administrative activities during scheduled time.
- Engage in continuous communication with patients and families to foster trust and understanding.
- Work collaboratively with the care team to ensure seamless patient experiences and high-quality care delivery.

Qualifications & Requirements
- MD or DO degree from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Bilingual skills, especially Spanish-speaking, are strongly preferred to best serve our patient community.
- Demonstrated commitment to providing compassionate, patient-focused healthcare to diverse populations.
- Strong interpersonal and communication skills for effective patient and team interactions.
