Primary Care Physician (MD/DO) - Summary
- Full-time position in a leading, mission-driven primary care practice
- Focus on providing care to seniors and those with multiple chronic conditions

- Comprehensive support staff, including Medical Assistant, Scheduler, and Referral Specialist
Duties & Responsibilities
- Provide high-quality, evidence-based primary care services to a panel of approximately 450 patients
- Manage care for seniors, focusing on individuals with 3-5 chronic diseases
- Conduct patient visits (18-22 per day) in a 100% outpatient setting
- Work closely with a dedicated care team to coordinate patient care, including in-house consulting specialists such as cardiology and podiatry
- Utilize holistic health services, including acupuncture, tai chi, yoga, and nutrition, to support patient wellness
- Participate in value-based care initiatives to enhance patient outcomes and satisfaction
- Maintain thorough and accurate patient records, ensuring compliance with all regulatory requirements
- Schedule includes administrative time allotted within working hours
- Contribute to a positive, team-oriented workplace culture that supports work/life balance with no nights or hospital call responsibilities

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Demonstrated commitment to value-based, patient-centered care
- Strong communication and interpersonal skills
- Ability to work effectively in a collaborative, multidisciplinary team environment
- Commitment to continuous professional development and learning
