Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to a Medicare patient population.
- Collaborate within a large, multidisciplinary care team.
- Practice in an environment focused on value-based healthcare and improved patient outcomes.
- Work with innovative technologies and resources to provide outstanding patient services.
Duties & Responsibilities
- Provide high-quality, patient-centered clinical care to assigned patients.
- Manage patient panel with a focus on preventive care and chronic disease management.
- Conduct patient visits (average 13–16 per day) with scheduled 20-minute regular visits and 40-minute diagnostic or new patient appointments.
- Collaborate with care team members, including scribes, medical assistants, and social workers.
- Utilize EMR platforms (Canopy and Greenway) to document patient encounters and coordinate care.
- Participate in care model initiatives to reduce hospital admissions and drive improved health outcomes.
- Access on-site laboratory and phlebotomy services for efficient patient workups.
- Engage in multidisciplinary meetings and value-based care program development.
- Support community engagement initiatives and patient wellness activities.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Bilingual abilities in Cantonese strongly preferred to serve patient population.
- Fellowship training in Geriatrics is a plus, but not required.
- No current medical residents eligible for this role.
