Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive outpatient primary care to a panel of senior patients with complex medical needs.
- Emphasize patient-centered, value-based care, ensuring optimal clinical outcomes and patient satisfaction.
- Be part of a new healthcare model that supports wellness, accessibility, and community engagement.
- Work within a collaborative, multidisciplinary care team to provide exceptional care and support for patients.
Duties & Responsibilities
- Provide direct outpatient care to seniors with multiple chronic conditions, such as heart disease, diabetes, and hypertension.
- Maintain a daily patient load of 10-12, with a maximum panel size of 650 patients to ensure quality and continuity.
- Participate in a value-based care model with incentives tied to patient satisfaction, adherence to clinical guidelines (HEDIS), and reducing hospitalizations.
- Collaborate closely with a dedicated care team, including medical assistants, schedulers, and referral specialists, for comprehensive patient support.
- Perform office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Engage in regular professional development and training to stay current with best practices in primary care and geriatrics.
- Work Monday through Friday, 8 a.m. to 5 p.m., with a call schedule based on the number of providers in the market.
- Document all patient encounters accurately and timely in the electronic medical record system.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted medical license and active DEA registration.
- Proficiency in office-based procedures, such as suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus but not required.
- Strong commitment to providing high-quality, patient-centered care.
