Primary Care Physician (MD/DO) - Summary- Full-time role in a value-based, outpatient care setting
- Focus on senior patient population with multiple chronic conditions

- Provide comprehensive outpatient primary care to a panel of seniors, managing 10-12 patients per day
- Utilize a value-based care model to improve patient outcomes and satisfaction
- Coordinate with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership
- Engage in activities that reduce hospitalizations and improve HEDIS compliance metrics
- Participate in a fair and predictable call rotation based on the number of providers in the market
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Contribute to initiatives that promote patient wellness, including educational and recreational offerings

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus but not required
- Current, unrestricted, active medical license
- Current DEA registration
- Proficiency in basic office procedures relevant to primary care
